How to find a medical provider:

Go to azblue.com
Select “Search the Provider Directory” at the bottom of the page
Select “PPO” and select “Next”
Search for your provider

How to find a dental provider:

Go to deltadentalaz.com
Select “Dentist Search”
Select “Delta Dental Premier Plan”
Enter in your and your provider’s information
Select “Search for a Dentist”

How to find a vision provider:

Go to vsp.com
Select “Member”
Select “Find a VSP Doctor” in the left column
Either “Register now” or select “Not a member?”
If you select “Not a member?”, select “Continue as a Non Member”
Enter in your information and the verification code
Select “Search”
